Met de App Risicoscan kunnen zorgmedewerkersin de ouderenzorg eenvoudig en snel gezondheidsrisico’s bij ouderensignaleren. De app geeft suggesties voor vervolgacties enpraktische adviezen om de zorg te verbeteren.Deze app voor risicosignalering, ontwikkeld door het KennispleinZorg voor Beter, is voor zorgmedewerkers in verpleeg- enverzorgingshuizen, en voor thuiszorgmedewerkers.Is er een verhoogd risico?Met de app beantwoord je vijf vragen voor degezondheidsrisico’s: ondervoeding, medicatiefouten, mondproblemen,huidletsel, incontinentie, depressie en vallen.
